The visit summary must be submitted within 24 hours of the end of the visit. To do so, please refer to the video guide and steps below:
- Go to "Upcoming" tab and look for the completed visit.
- Click on "Submit Visit Summary" or "Submit Visit Summary and Claims".
- Provide details of the visit and be as detailed as possible:
A. For Visit Summary:
- Clock-in & clock-out time: Specify if there were time extensions
- Services Provided: Detail the services you provided during the visit and indicate if the Care Recipient (CR) declined engagement.
- You can also select if you performed any other additional service(s) that was not listed in the initial visit instructions
- Complex Nursing Procedures only: For Staff Nurse or Medical Assistants, inform CO/CR that there may be extra charges for each additional complex nursing procedure (Note: additional payout to CP will be subjected to Homage and CO's approval)
- Care Summary: Provide a detailed Care Summary with at least 50 words to share detailed information about the visit with family and fellow Care Pros assisting the same CR. Detail the visit on a time-stamp basis, including all activities conducted. If CR refused to engage, indicate this objectively. Refer to the tip section for an example
- Vital Signs: Include if there was any vital signs measured
- Health & Wellbeing: Report on CR's alertness level and pain intensity if CR expressed any pain or discomfort.
- Photo/file attachments (Recommended): All photographs require a description. PDF files can be uploaded as well.
- Read the visit instructions carefully if photos were requested by the CO
- Seek consent - before taking any pictures of CR and avoid uploading any inappropriate photos of CR e.g. naked without censoring private parts)
- Video attachments are not possible
B. For Claims: Claim items will be reviewed by Care Owner and Homage so ensure you provide accurate information and within 24 hours from the visit end time.
- After filling in the details above, you will be able to review your visit summary. Please vet through your visit summary to ensure that it is thorough and detailed. You can then proceed to click on the 'Submit Visit Summary" button.
- If there has been an incident or you have any concerns, please inform Homage by detailing the incident and/or concern on the Incidents/Concerns screen in-app after Visit Summary submission.
If this is your first visit summary, it will be reviewed by Homage before sharing to Care Owners & fellow Care Pros assisting Care Recipient.
